You may use the following form to request an insurance verification letter to send to your insurance company.

By sending this request, you agree to pay the $25.00 fee for this service; you will receive an invoice in the mail separately from the requested letter from the Board of County Commissioners. There is routinely a 10-14 day wait as Fire Rescue receives many letter requests. All requests are handled in the order in which they are received.

All information on the form is required in order for us to process this request. Incomplete or invalid entries will be ignored.
